This Privacy Policy explains how KNWN Health Nursing Corporation ("We," "Us," or "Our") collects, uses, and protects information when you use our website and telehealth platform (the "Platform").

About Us

KNWN Health Nursing Corporation is a California Professional Corporation that provides telehealth medical services directly to patients on a cash-pay basis. We are the HIPAA Covered Entity for all clinical services delivered through this Platform. This Privacy Policy covers our general data practices and our handling of Protected Health Information ("PHI") under applicable law. Information We Collect

We may collect the following categories of information:

  • Contact information (name, email address, phone number, mailing address)
  • Account and login credentials
  • Appointment scheduling and visit information
  • Payment and billing information (we do not store full card numbers)
  • Health and medical information submitted through intake forms, questionnaires, or clinical communications
  • Technical data (IP address, browser type, device information, usage logs)
  • Communications you send through the Platform

Health information collected as part of clinical care is PHI governed by HIPAA and Our Notice of Privacy Practices.

Health Information and Intake Forms

When you submit information through intake forms or clinical onboarding tools on the Platform, you may provide health-related information, including medical history, symptoms, and treatment preferences ("Health Information"). This information is used by KNWN Health Nursing Corporation and its licensed clinicians solely to provide and coordinate your clinical care. Health Information becomes part of your medical record and is governed by HIPAA, the California Confidentiality of Medical Information Act ("CMIA"), and Our Notice of Privacy Practices.

How Information Is Used

Information we collect may be used to:

  • Provide, coordinate, and document clinical services
  • Schedule and manage appointments
  • Process payments
  • Communicate appointment reminders and administrative updates
  • Send prescriptions to licensed pharmacies
  • Comply with legal, regulatory, and licensing requirements
  • Improve Platform functionality and security
  • Conduct quality improvement and staff training activities

HIPAA and Protected Health Information

KNWN Health Nursing Corporation is the HIPAA Covered Entity for all clinical services. PHI created or maintained in connection with your care is governed by HIPAA and Our Notice of Privacy Practices. We implement administrative, technical, and physical safeguards to protect PHI in accordance with HIPAA and the California CMIA.

Sharing of Information

We do not sell your personal information. We may share information in the following circumstances:

  • With licensed clinicians and clinical staff involved in your care
  • With licensed pharmacies to fulfill prescriptions
  • With payment processors and billing services
  • With technology vendors and service providers supporting Platform operations, under Business Associate Agreements where required by HIPAA
  • With public health authorities or government agencies as required by law
  • In response to lawful legal process, court orders, or regulatory inquiries

Service Providers

We may engage third-party service providers for hosting, scheduling, payment processing, and analytics. Where required by HIPAA, we execute Business Associate Agreements to ensure appropriate protection of PHI.

Cookies, Tracking, and Your Opt-Out Rights

The Platform uses cookies and similar tracking technologies to support functionality, analyze usage, and improve your experience. The following types of cookies may be used:

  • Strictly necessary cookies: required for the Platform to function. These cannot be disabled.
  • Functional cookies: remember your preferences and settings.
  • Analytics cookies: help us understand how the Platform is used (e.g., Google Analytics). These may involve the transfer of data to third parties.
  • Marketing or advertising cookies: we do not currently use advertising cookies. If this changes, this Policy will be updated.

Your Cookie Choices

You may limit or disable non-essential cookies through your browser settings or a cookie preference center available on the Platform. Disabling certain cookies may affect Platform functionality.

California Privacy Rights — CCPA / CPRA

California residents have the following rights under the California Consumer Privacy Act (CCPA) and California Privacy Rights Act (CPRA):

  • Right to Know: request disclosure of the categories and specific pieces of personal information we have collected about you.
  • Right to Delete: request deletion of personal information we hold, subject to legal exceptions.
  • Right to Correct: request correction of inaccurate personal information.
  • Right to Opt Out of Sale or Sharing: we do not sell or share personal information for cross-context behavioral advertising. If this practice changes, we will provide a "Do Not Sell or Share My Personal Information" link on our homepage.
  • Right to Limit Use of Sensitive Personal Information: you may request that we limit the use of sensitive personal information to purposes necessary to provide our services.
  • Right to Non-Discrimination: we will not discriminate against you for exercising any CCPA/CPRA right.

Note: health information governed by HIPAA or the CMIA may be exempt from certain CCPA/CPRA rights. To submit a privacy rights request, contact us using the information below. We will respond within 45 days as required by law.

Data Security

We implement reasonable administrative, technical, and physical safeguards to protect personal and health information. However, no electronic system can guarantee absolute security. Please use secure, private networks when accessing the Platform.

Data Retention

We retain personal information and medical records for the period required by applicable California law, including California Business and Professions Code requirements for medical record retention (minimum 7 years from the date of service, or longer for minors). Payment records are retained as required by applicable tax and financial laws.

Children's Privacy

This Platform is not directed to individuals under the age of 18. We do not knowingly collect personal information from minors without the consent of a parent or legal guardian. If you believe a minor's information has been submitted without authorization, please contact us immediately.

Changes to This Policy

We may update this Privacy Policy periodically to reflect changes in our practices or applicable law. Updates will be posted on this page with a revised effective date. Continued use of the Platform after the revised effective date constitutes acceptance of the updated Policy.
